Clinical Pharmacy & Pharmaceutical Care
Objective:
The second month of the apprenticeship emphasizes the development of clinical pharmacy and pharmaceutical care competencies. Students actively engage in ward-based clinical rounds with physicians and healthcare teams, where they assess patient cases, monitor pharmacotherapy, and contribute to therapeutic decision-making. They are trained to identify and resolve drug-related problems (DRPs), perform dose adjustments in renal and hepatic impairments, and monitor drug-drug and drug-disease interactions. Rotations through specialized hospital units—such as cardiology, pulmonology, pediatrics, dermatology, psychiatry, and gynecology—allow students to apply pharmacological knowledge to real-world patient care. A strong focus is placed on pharmaceutical care planning, including patient assessment, follow-up, and adherence strategies. Students are also encouraged to participate in clinical case presentations and mini research projects to strengthen their critical thinking, data analysis, and evidence-based practice skills. Throughout the second month, interpersonal and communication skills are further refined through direct patient counseling and collaboration with multidisciplinary healthcare teams. Ethical and legal aspects of patient care are integrated into daily practice, reinforcing the professional responsibilities of future clinical pharmacists..
